Here are a few online options - I've only actually used ponoko (have a
lightly used prime account), but have interacted with each of the
other sites and/or and/or virtually know people who have used them
with positive results:

purely custom work, you must e-mail them and get a quote

big blue saw
http://www.bigbluesaw.com/
fully automated system - 5 of anything is usually the same price as 1
of anything

ponoko (takes svg files)
http://www.ponoko.com/
mostly automated system, but you must sign up as a "seller" - not a
big deal, though, and cheap

emachineshop
http://www.emachineshop.com/
free downloadable software - probably better for larger scale - has
more machining options, but seems to have longer turnaroun/more
expensive
